The Dimensional Obsessive-Compulsive Scale assesses obsessive-compulsive symptoms across four areas: fear of contamination, concern about responsibility and causing harm, unacceptable thoughts, and the need for symmetry. It makes visible how much time each area takes up and how much distress it causes.

What does the DOCS measure?

The scale treats obsessive-compulsive symptoms across four common dimensions, independent of the content of the symptom. This way, whatever the type of symptom, it can be compared in terms of how much time it takes, how much avoidance it leads to and how much it affects daily life.

Seeing which dimension stands out helps to set the focus of a plan of work.

How is it scored?

In each dimension, time, avoidance, distress and difficulty with control are rated separately and summed. The total score falls into below-clinical, mild-to-moderate and severe ranges.

What do the score bands mean?

The ranges below are a general frame. No band makes a diagnosis on its own; a result only becomes meaningful when a clinical psychologist interprets it alongside your life story.

Limitation: It relies on self-report and a scale never makes a diagnosis on its own. An OCD diagnosis and differential assessment require an evaluation with a specialist; some symptoms, such as hoarding, are measured separately.

Psychometrics and source

Developed by Abramowitz and colleagues (2010); with its dimensional structure and its measurement of severity independent of symptom content, it shows strong psychometric properties in contemporary OCD assessment.
